Festival Vendor Form

For participation in this event you MUST:
• Complete and Sign Vendor Application Form
• Upon receipt of approval as a Funny Farm Festival Vendor, payment will be due at that time
• Vendor Payment can be made Online via PayPal/Credit Card, or Check
• Set up begins at 6:30 a.m. and must be completed by 8:30 a.m.
• Agree to sell ONLY the items approved by Funny Farm Rescue (Food must be MEATLESS!)
Upon approval, to participate in this event, you will need to provide copies of the following:
• Current certificate of Insurance showing Liability and Auto
• Current Board of Health certificate
• Township of Hamilton Fire Permit (if cooking vendor)
